Your First Visit
Please bring the following to your first appointment:
- All of your insurance cards
- Photo ID
- A list of all the medications you are currently taking, along with the medication bottles
- All of your medical records. Please ask your family physician to send your records to our office prior to your first appointment
- A list of any questions you would like to ask the doctor
During your first appointment, your physician will review your medical records, perform a physical examination, and answer any questions you may have. Your doctor will then determine if additional testing is needed. Please allow at least one hour for your first visit.
Your Next Appointment
Please bring your insurance cards and medication bottles with you to every appointment. This ensures that your medical information is always up-to-date and accurate. If you choose to bring a list of your medications, please include the strength and number of times per day the dosage is taken.
We invite your spouse or close relation to accompany you to appointments. However, if you are scheduled for a procedure where they cannot be admitted, we will notify you ahead of time.
It is very important to us that you are able to see the same doctor each time you visit the office. Although there will be times when your physician is not available, we will make every effort to ensure that your medical care is handled by your physician every time.
